New Patient Registration

If you live in the practice designated area (see below) you can join the list. You will need to collect a new patient registration form and pack from Reception at either our Tutbury surgery or Sudbury surgery, or download from the links below;

GMS1 form; GMS1 Family Doctor Registration Form (THIS MUST BE COMPLETED AS WELL AS NEW PATIENT FORMS)

New Patient (Child); New Patient Form

New Patient (Adult); New Patient Form 16+

Any patient over the age of 16 needs to bring in their own forms with photo ID and proof of address.

Which surgery of the Dove River Practice you register with will depend on where you live:

At the Tutbury Surgery we see patients from the following villages:

Hatton
Hilton
Marston-on-Dove
Rolleston
Stretton
Sutton-on-the-Hill,
areas of Outwoods. 

At the Sudbury Surgery we provide an additional dispensing service and normally see patients from the following villages:

Alkmonton
Anslow
Boylestone
Church Broughton
Coton-in-the-Clay
Cubley
Doveridge
Draycott-in-the-Clay
Fauld
Foston
Hanbury
Marchington
Newborough
Scropton
Somersal Herbert
Sudbury.

Change of Patient Details

If you change name, address or telephone number etc, please let us know as soon as possible. We have a change of address/ contact details  form available at reception. 

If you move outside of the practice area it will be necessary for you to find a doctor in your new area.
